Parents feel shock, anxiety and the need to protect children with genital ambiguity

ScienceDaily (Sep. 28, 2011) ? Parents of babies born without clearly defined male or female genitals experience a roller-coaster of emotions, including shock, anxiety and the need to protect their child, according to a study in the October issue of the Journal of Advanced Nursing.

UK researchers who spoke to 15 parents found that they were keen to find a sense of harmony between their child's genital ambiguity and the sex they raised them as.

"The parents we spoke to went through a dynamic and evolving process, which included their willingness to engage with professionals" says Dr Caroline Sanders, a Consultant Nurse working in paediatric urology and gynaecology at Alder Hey Children's NHS Foundation Trust in Liverpool.

"They had to deal with challenges relating to their lives, emotions and beliefs and one of the mothers, Faye, experienced serious abuse because people thought her daughter was different. At the other end of the scale, Imogene was delighted when a scan showed that her child, who she felt looked like a girl, was female inside."

Dr Sanders teamed up with Professor Bernie Carter and Dr Lynne Goodacre from the University of Central Lancashire to carry out the study, which involved in-depth interviews with ten mothers and five fathers over a period of 18 months. Two of their 11 children -- six girls and five boys -- were under four and the remainder were aged five to 11.

All the children had disorders of sex development, which include conditions where the chromosomes, testicles, ovaries or sexual anatomy are not as expected. It's estimated that one in 300 babies are born with concerns about the development of their external genitalia and in one in 5,000 births the baby's sex is unclear despite expert examination.

Parents who took part in the study said the events following their child's birth were "confusing" and "chaotic" and led to bewilderment and loss of orientation. Several parents recalled that some healthcare professionals had been vague or hesitant when discussing their child's genital uncertainty, which heightened their anxiety. This was compounded when healthcare professionals referred to their genderless child as "it," inferring that their child was "wrong" or a "freak."

Many of the parents protected their child by not sharing news of their child's condition with extended family or friends. Information was kept private, but not secret, while parents tried to understand what was happening. "You couldn't explain it because you didn't really know" said Gina. Maria felt the need to be open about it, but adds that: "My husband wouldn't have told a soul?because it is genitalia and is still looked at as something that's taboo." Brian, however wanted to talk about it. "I found it easier talking to people about it" he said.

Faye found local people's interest in her baby cruel, intrusive and alarming. She recalled people stopping her in the street, pulling the covers off her baby and saying "oh you'd never know would ya?"

Early surgical decisions raised strong emotions in some parents. Sian was anxious about whether she had done "the right thing," but Anne described a documentary that suggested that parents shouldn't be allowed to make decisions about genital surgery as "ridiculous."

Some parents were concerned about whether they had made the right decision about their child's sex and subsequent gender and worried about their children feeling different to others and experiencing uncertainty in adolescence. They wanted to protect their child, but at the same time they felt that they needed to be honest with them as they grew up.

Reconstructive genital surgery made it easier for some parents to protect and bond with their child. Medical evidence about whether the child was predominantly male or female, and how they looked, guided the parents' decisions when it came to surgery. But one mother who learnt that her child had both male and female internal organs described the news as a "double whammy."

The parents also needed healthcare professionals to use consistent and understandable language to explain what was happening to their child and how they could move forward. However, in some cases there were no clear-cut answers, particularly when the conditions were caused by chromosomal abnormalities.

"Our study revealed the serious emotional traumas and dilemmas that having a child with ambiguous genitalia creates for parents" says Dr Sanders. "It underlines the need for greater sensitivity and understanding about the issues these parents face.

"Health professionals need to be aware of the impact that medical language has on parents, particularly in the early stages, and communicate with them clearly and regularly, checking that they understand what they are being told.

"We also feel that further resources are needed to support parents and children affected by these disorders, including the development of a national network -- building on the work already done in Scotland -- and integrated working to improve standards of care."

Structured settlements provide long term income to families or individuals who have law suits pending due to injury or illness. Poor critical care 'risking lives' - Health Nutrition and Fitness

Thousands of patients needing emergency surgery are having their lives put at
risk by poor NHS care and delays in accessing treatment, according to a
damning report.

The Royal College of Surgeons study found that only a minority of patients who
need critical care following surgery receive it, while some die or suffer
major complications because of delays in finding space in operating theatres.

Junior staff are often left in charge of dealing with post-surgical
complications, which can rapidly lead to death if not treated promptly, the
report went on.

A patient?s chance of survival also varies widely between NHS hospitals, and
even within the same hospital depending on the day of the week.

The report calls for the NHS to improve the way it deals with this group of
?forgotten? patients, who are often elderly.

Some 170,000 patients have major emergency surgery each year, mostly on the
abdomen.

Of these, 100,000 will develop significant complications following surgery,
resulting in more than 25,000 deaths.

In the UK, fewer than one in three of all these patients are admitted to
critical care following their surgery.

Even those who are admitted only tend to stay 24 hours before enduring a
longer hospital stay on other wards.

The report said: ?Premature discharge from critical care has been identified
as an important risk factor for post-operative death, as has delayed
admission to critical care.?

On managing complications, the study added: ?Too often the whole process is
slow or inaccurate as it is complex, requires multidisciplinary input, often
occurs out of hours and is initiated by junior staff.?

Surgeons leading the study also pointed to ?suboptimal care on general wards?
as a factor in poor outcomes following surgery.

Research highlighted in the report shows that the chance of a patient dying in
a UK hospital is 10% higher if they are admitted at a weekend rather than
during the week.

?There are no evident reasons for these differences other than that care, at
times, is of variable quality: a conclusion which fits with the available
evidence and professional opinion,? it added.

An analysis of several patients who died showed some suffered from delays in
assessment, decision making and treatment.

?There were shortfalls in access to theatre, radiology and critical care;
surgery was suboptimally supervised in 30% of cases and there was a failure
for juniors to call for help in 21% of cases,? the study went on.

?Timely surgery was not carried out in 22% of patients who died.?

In general, there seems to be a lack of appreciation across the NHS of the
level of risk for emergency surgical patients, the report said.

Death rates of 15% to 20% are typical and rise as high as 40% in the most
elderly patients.

But this imminent risk of death is not being reflected in the priority given
to these patients, whose chances of survival can more than double, depending
on which NHS hospital they are treated in.

Recommendations in the study include improving access to operating theatres to
overcome the fact delays are ?common?.

The study says hospitals should provide fast access to operating theatres
within defined time periods and prioritise emergency cases over planned
surgery wherever necessary.

It may be that separation of planned and unplanned operations is necessary.

Another recommendation is for the highest risk patients to be treated under
the direct supervision of consultant surgeons, anaesthetists and intensive
care staff, while more needs to be done on access to critical care.

Iain Anderson, report author and consultant general surgeon at Salford Royal
NHS Foundation Trust, said: ?Complications and death rates vary
significantly between hospitals and even within the same hospital depending
on the time of admission.

?Trusts should acknowledge that these problems exist and work to review their
services using this guidance.?

Norman Williams, president of the RCS, said: ?The focus on reducing waiting
times for elective procedures has resulted in a large group, of mostly
elderly patients, becoming seriously under-prioritised to the point of
neglect in the some NHS hospitals.

?These changes won?t happen on their own and we are calling on all surgeons
and managers to work together to deliver the high quality care that these
patients need and which some hospitals are already proving can be delivered.?

try this , there is only 3 things in it, use any bbq sauce you want, but i have not tried diet coke, but that would be less calories.

INGREDIENTS:
Chicken (boneless, skinless breasts or pieces)
1 cup BBQ sauce
1 cup coke
PREPARATION:
Place chicken in the pot and add one cup of your favorite bbq sauce and 1 cup of coke (plain old Coca-Cola). Cook on low for 8 to 10 hours. The carbonation in the coke tenderizes the meat. I also use this combination with butterfly pork chops.

My P.E. teacher wanted to major in something concentrating in physical therapy or sports medicine. I think he chose Kinesiology? (Sorry, I?m not the best speller!) I would ask your counselor if you?re still in HS, or maybe talk to a master dean or some other academic advisor at your school. How much physical therapists earn would depend on their employer or where they work (at a HS, or as a private gym trainer, for a hospital, etc.)PTs work mostly with people with injuries. It is a clinical doctorate. It takes 6-7 years after hs. www.apta.org.

What you describe is a personal trainer. They do not even need a degree just passing a weekend certification course. They make very little money.Great question. To answer simply, they are different degrees. Health and Physical Fitness likely would get you a job as a personal trainer or an aide in a physical therapy facility. These individuals make very little money.

To practice physical therapy, you will need an advanced graduate degree, most entry-level programs offer a doctorate, as well as clinical experience. A physical therapist is licensed by the state in which they are practicing and are experts in ?movement?. In many states, they are able to practice without referral, and are able to diagnose movement dysfunctions (for example, if you are running and your ankle hurts, you would go to a physical therapist to determine why it hurts and how to get it to move correctly so that you can run without pain). Physical therapists do well but pay varies based on specialization as well as the state in which you practice. Franz Nopcsa was a turn-of-the-century baron of Szacsal in Transylvania who discovered some of the first dinosaurs from central Europe. His ideas about fossil analysis and dinosaur evolution were remarkably prescient, as this article?in the October issue describes. Below is a timeline of major events in the dinosaur baron?s life, some of his paleontological discoveries and recent work that has validated his claims. A stabilization trend is taking place in commercial real estate sectors, but in most markets rent will remain soft except for multifamily rentals, according to the National Association of Realtors?.

Lawrence Yun, NAR chief economist, said a pullback in construction is helping stabilize the market. ?Very limited construction of new commercial real estate over the past few years has essentially fixed the supply of available space,? he said. ?This means vacancy rates could fall quickly from any increase in demand for commercial space.?

From the first quarter of this year to the first quarter of 2012, NAR expects vacancy rates to decline 0.5 percentage point in the office sector, 1.3 points in industrial real estate, 0.1 point in the retail sector and 0.9 percentage point in the multifamily rental market.

?Even with declining vacancy rates, rents are not likely to turn positive in most markets until next year, outside of multifamily rental properties,? Yun said. For example, office rents are forecast to fall 1.8 percent this year before turning higher by 4.0 percent in 2012.

?Apartment rent increases are expected to accelerate from job creation leading to new household formation, particularly among the young adult population who will seek their own housing arrangements ? many will be leaving their parents? homes, or choose to live with fewer roommates,? Yun said.

Average apartment rent is projected to grow 3.4 percent this year and another 4.2 percent in 2012.

?Rising apartment rent in combination with rising oil prices could push the overall inflation rate beyond a comfort level, which could then force the Federal Reserve to raise interest rates later this year or early in 2012,? Yun added.

The Society of Industrial and Office Realtors?, in its SIOR Commercial Real Estate Index, an attitudinal survey of more than 360 local market experts¹, shows a notable improvement in market fundamentals.

The SIOR index, measuring the impact of 10 variables, rose 8.1 percentage points to 50.7 in the fourth quarter, the largest quarterly gain in five years, and is at the highest level since the fall of 2008. However, the index is well below a level of 100 that represents a balanced marketplace. This is the fifth consecutive quarterly improvement following nearly three years of decline, but the last time the index was at the 100 level was in the third quarter of 2007.

Seventy-eight percent of SIOR participants expect improvements in the office and industrial sectors for the first quarter of this year.

There has been an increase of liquidity in Commercial Mortgage Backed Securities, which is helping to open the commercial market to more property transactions; commercial real estate sales had been stalled over the past few years with excessively tight credit conditions. In terms of development acquisitions, it remains a buyer?s market for those with cash or who can obtain credit financing.

NAR?s latest COMMERCIAL REAL ESTATE OUTLOOK² offers projections for four major commercial sectors and analyzes quarterly data in the office, industrial, retail and multifamily markets. Historic data were provided by CBRE Econometric Advisors.

Office Markets
Vacancy rates in the office sector are forecast to decline from 16.5 percent in the first quarter of this year to 16.0 percent in the first quarter of 2012.

The markets with the lowest office vacancy rates currently are New York City and Honolulu, with vacancies in the 8 to 9 percent range.

In 57 markets tracked, net absorption of office space, which includes the leasing of new space coming on the market as well as space in existing properties, should be 14.5 million square feet in 2011.

Industrial Markets
Industrial vacancy rates are projected to decline from 14.2 percent in the current quarter to 12.9 percent in the first quarter of 2012.

At present, the areas with the lowest industrial vacancy rates are Los Angeles and Salt Lake City, with vacancies of 7.5 percent.

Annual industrial rent is likely to decline 2.5 percent in 2011, before rising 3.0 percent next year. Net absorption of industrial space in 58 markets tracked should be 127.5 million square feet in 2011.

Retail Markets
Retail vacancy rates are expected to slip from 13.0 percent in the first quarter of this year to 12.9 percent in the first quarter of 2012.

Markets with the lowest retail vacancy rates currently include San Francisco; Miami; Honolulu; and Long Island, N.Y., all with vacancies in the 7 to 8 percent range.

Average retail rent is seen to decline 0.9 percent in 2011, then rising 0.7 percent next year. Net absorption of retail space in 53 tracked markets is projected to be 4.8 million in 2011.

Multifamily Markets
The apartment rental market ? multifamily housing ? is tightening as the economy improves. Multifamily vacancy rates are forecast to decline from 5.8 percent in the current quarter to 4.9 percent in the first quarter of 2012.

Areas with the lowest multifamily vacancy rates presently are San Jose, Calif.; Pittsburgh; and Newark, N.J, with vacancies in a range around 3 percent.

Multifamily net absorption should be 207,000 units in 59 tracked metro areas in 2011.

India?s overall score was only 41.9 out of 100; with only 14.8 in politics, 54 in justice, 60.7 in economics, 64.1 in health and 64.9 in education. Zed, who is the President of Universal Society of Hinduism, argued that although India was on track to become a global power, but her new power and prosperity had remained evasive for many, especially women. Despite economic miracle, women in India continued to face inequalities in opportunities which blocked them from fully participating in the growth process. It was blight on a country, which prided herself on having joined the league of hottest growth economies.

Libya NTC says to announce "crisis" government in days (Reuters)

BENGHAZI/SIRTE, Libya (Reuters) ? Libya's interim rulers said on Friday they would announce a "crisis" government within the next few days, signaling a breakthrough in efforts to form a more inclusive administration after the war that ousted Muammar Gaddafi.

"We've agreed on a number of portfolios and who would hold the most important ones. There will be 22 portfolios and one vice premier," said Abdel Hafiz Ghoga, a spokesman for the National Transitional Council (NTC).

"It will be a compact government, a crisis government."

NTC forces said they had tightened their grip on southern oasis towns which sided with Gaddafi. That progress is overshadowed by unsuccessful efforts to take two remaining strongholds loyal to the ousted leader.

It remains unclear whether the NTC, still based in the eastern city of Benghazi, can unify a country split along tribal and regional lines.

Getting the oil on which the economy relies pumping again is key to building the NTC's credibility and earning more of the cash it needs to build a new state after 42 years of eccentric, one-man rule.

Those efforts were boosted on Friday when France's Total said production at its Al-Jurf offshore field had restarted and that crude shipments were likely to resume in around two weeks.

The company said it still needed to carry out detailed inspections at its onshore sites before being in a position to say when it would restart production. Total has a Libyan crude production capacity of 55,000 barrels per day.

The announcement is the latest step toward getting oil pumping at full capacity again, though wresting control of Bani Walid and Sirte from Gaddafi's forces remains the NTC priority.

"RESISTANCE IS HOPELESS"

A Reuters reporter on the western edges of Sirte saw dozens of cars with civilians leaving the town on Friday. Rebels fired sporadic tank shells and artillery at suspected positions of Gaddafi loyalists. NATO aircraft could be heard overhead.

"In the city, as soon as you leave the main square there is shooting. It is an effort to scare the residents," said Massoud al Adawi, a fleeing resident of Sirte.

"Gaddafi loyalists) don't want people to leave the city. They want to use them as human shields."

Amr al-Aswar, a NTC military commander on the western edge of Sirte, said avoiding civilian casualties in the town was the main problem for his forces.

Until Thursday, some parts of Sabha, the traditional base for Gaddafi's own tribe about 800 km (500 miles) south of Tripoli, had been occupied by fighters loyal to the leader who lost control of the capital and most of the country last month.

"Our revolutionaries are controlling 100 percent of Sabha city, although there are some pockets of resistance by snipers," NTC military spokesman Ahmed Bani said on Thursday in Tripoli.

"This resistance is hopeless ... They are fighting for themselves, not for the tyrant," he told reporters.

The NTC says it also controls Jufra, to the northeast of Sabha, and the nearby oasis towns of Sokna, Waddan, and Houn.

A manhunt for Gaddafi, who has been in hiding for weeks occasionally issuing defiant audio messages through Syrian-based Arrai TV, was drawing closer to its target, said Bani.

The network broadcast a bad-tempered audio message from Gaddafi's daughter Aisha on Friday. It was the first time she had spoken publicly since fleeing into exile in Algeria last month along with two of his sons.

"I reassure you about your leader, oh Libyans," the 35-year-old lawyer said, sounding distressed. "He is well and thank God his spirits are high. He is carrying his weapon and fighting along with his sons at the fronts."

A spokesman for Gaddafi, Moussa Ibrahim, said on Thursday NATO air strikes and interim government forces' shelling of Sirte were killing civilians.

His statement could not be verified as journalists are unable to reach the city. NATO comment was not immediately available.

Interim government fighters near Sirte and residents fleeing the city said pro-Gaddafi forces had been executing people suspected of sympathizing with the NTC.

North of Bani Walid, NTC military forces brought forward tanks and Grad rocket launchers for a renewed attempt to take the town although it was not clear when the attack might begin.

The offensive there has been frustrated by stiff resistance from well-drilled loyalist fighters, and also a lack of organization among the attacking forces. They operate in disparate units based on their home towns, with little overall command.

Many fighters go into battle wearing flip-flop sandals, t-shirts and jeans, and have no military training.

"We don't take orders from the NTC. We listen only to our own commander," said Ziyad Al Khemri, a fighter from Zawiyah, just west of Tripoli.

If the NTC cannot swiftly impose control on the country and its own forces, this may embarrass Western leaders, especially France's Nicolas Sarkozy and Britain's David Cameron, who took a gamble by backing the anti-Gaddafi leadership.

The NTC said last week it would move to Tripoli only after its forces were in full control of Libyan territory, contradicting an earlier pledge to move the interim administration to the capital around mid-September.

"Complete liberation would be announced when we are in control of Sirte and Bani Walid and control all the border crossings," said NTC spokesman Ghoga.

"This means Gaddafi forces would have no control over any of those crossings. I believe it's a matter of few days."
